To improve behavioral competence, perceived quality of life, objective environment, and psychological well-being of older persons, we have been working on three research themes. First, we explore measures for quality of home care that represents three perspectives: support to older persons, support to caregivers, and support to the community. Second, we explore a program that ensures care coordination and quality care for frail older adults during transitions between home, hospital, and long-term care settings. Finally, we identify a care system that is necessary for older adults to live out their final days with their dignity preserved.
